You will be redirected to login at https://login.wisc.edu/, a site maintained by DoIT, the purpose of which is to authenticate your identity.  Please use your UW NetID and password to login, the same information you use to access your email at wiscmail.wisc.edu. 

You must login to verify that you are in the course and to prevent duplicate responses. None of your personal information is associated with the course evaluation when sent to instructors, and evaluations will not be released until after grades have been processed. Faculty are not notified regarding who has or has not filled out an evaluation, nor do they have access to our department's web server. This process is even more anonymous than the paper evaluations in that it eliminates the possibility of someone recognizing your handwriting.

As your responses are utilized to improve MMI courses, we ask that you express your thoughts in as complete and detailed a manner as possible.  The submission deadline is Monday, May 19, 2008 at 8:00 am.
