    You will be asked to provide a brief personal statement. You may want prepare it in a word processing program and save a copy in case submission of this form fails.

  2. A UW transcript. After you submit this application, you will be given an opportunity to download a transcript request form.

Please list the grades you have received in the following UW-Madison college chemistry and biology courses.
  • Our policy regarding transfer and AP credits is the same as the UW at large. Transfer students should enter the grade they received in the respective course at their former institution. Only courses already approved by the Admissions Office will be accepted as directly equivalent to those below.

  • Students who took the AP Chemistry exam and received a score of 4 or 5 would have received 3 credits towards Chemistry 103. Please enter B if you received a 4 and A if you received a 5 for this course.

  • AP Biology credits cannot be substituted for any UW biology courses. However, if students are permitted to take Zoology 152 as a result of a high school AP biology score of 4 or 5, the grade for Zoology 152 will count toward the admissions GPA and biology requirement for the MMI major. If you have taken both Zoology 151 and 152 at the UW, please list your grade for only Zoology 151, as that will be the only course considered. Do not attempt to average both grades.
