The following application materials are required for consideration for admission to the MMI major.

1. The application form (Microsoft Word, HTML format, or Adobe Acrobat PDF).
2. A UW transcript, which can be obtained from the Registrar at Rm 123 Peterson Building, 750 University Ave.
3. A personal statement not to exceed one page that includes such information as "how and why you became interested in MMI as a major" and "what your major will contribute to your future".
4. Please include a current email address or a stamped, self-addressed envelope to be used to notify you of your admission status.
